Taylor's Hope 5k

WHAT: A 5k fundraiser. Runners, walkers, and casual strollers are all welcome :)

WHEN: Saturday, May 15th, 2010. The 5k starts promptly at 8:30am. The kids race starts at 8:15am.

WHO: This 5k is being held in honor of Taylor, a 4 year old little girl bravely battling Spinal Muscular Atrophy.

WHERE: James River High School, 3700 James River Road, Midlothian VA 23113. Both races (the kids and the 5k) start on the track located at the football field.

COST: The registration fee is $20 and is not refundable. Please fill out the registration form below for each person participating in the 5k. T-shirts are guaranteed to those who register on, or before, Saturday May 1st. A raffle ticket is included in each participants runner packet, and more can be purchased at packet pickup.

PAYMENT: Please pay via Paypal using the options at the bottom of this form. If you prefer to send a check by mail, please make the check payabe to "Taylor's Hope", and send it to: PO BOX 1352, Midlothian VA 23113

PRIZES: A CASH prize of $75 will be awarded to the overall male & female winner. Raffle drawings will be held at the end of the race for prizes and gift certificates - you must be present to win!

HOW YOU ARE HELPING: Proceeds from this event will be going towards Taylor's semiannual medical trip to her specialist in Salt Lake City, and towards her biweekly Physical Therapy expenses. A percentage will also go to FightSMA - a local organization supporting research to find a treatment or cure for SMA.

We will be having a race packet pick-up on Friday, May 14th from 4-6. This will be located in front of the James River High School football field.

Will you be picking up your packet on Friday, or prior to 8:45am on Saturday?
